Netilmicin and Piperacillin were found to be the most effective drugs for S. aureus and P. aeruginosa.

OD Dose Provides:

Minimal renal cortical uptake and thus decreases nephroloxicity
Low perilymph levels results in lesser incidence of ototoxicity
Good Post antibiotic effect which suppresses bacterial regrowth even at sub MIC drug level

Indications:

Dosage:

Adults Complicated UTIs	4 to 6 mg/kg/day
Serious life threatening infections	6 to 7.5 mg/kg/day
Children (6 weeks through 12 years. of age)	6 to 7.5 mg/kg/day
Premature of full term Neonates (<1 week of age)	6 mg/kg/day
Neonates over one week of age	7.9 to 9 mg/kg/day

Maintenance Dose every 8 hours = Observed CCr* / Normal CCr x Usual Maintenance Dose

CCr = Creatinine clearance rate in mL/min/1.73m².
